Information From Your Employer
San Bernardino City new hires please make sure to schedule your new hire benefits appointment. Your enrollment must be complete within 30 days of your hire date. If you will be adding a dependent to your medical, dental, vision benefits you must have documentation with you at the time of your appointment.
Spouses- 2024 (1040) tax form including signature page
Children- birth certificate
Domestic partner- certificate of registered domestic partnership
